COPAYMENTS

If your insurance plan requires a copayment, it will be due at the time of your visit. Acceptable methods of payment are cash, check, and all major debit and credit cards (including HSA and FSA).

Cosulich Dermatology is a provider for Medicare and most major insurance plans. Please note that patients are responsible for copayments or charges that are not covered by the insurance plan. If you have questions regarding billing or which insurance plans we accept, please contact us at (732) 280-1200. With any questions regarding insurance coverage and your specific benefits, please contact your employer or insurance provider.
